Records Notice

In accordance with HIPAA guidelines, CCNETX provides individuals access to their protected health information (PHI) free of charge. We do not charge fees for electronic or paper copies, postage, or summaries of your records.

Please note:

  • There may be circumstances, such as open criminal investigations, that would prohibit CCNETX from granting access to your records. You will be notified if we are unable to comply with your request.
